Job Description
Matrix Medical Network seeks a Nurse Practitioner to deliver in-home health assessments and ongoing clinical care for diverse patient populations. In this field-based role, you will conduct comprehensive evaluations, diagnose and manage acute and chronic conditions, prescribe medications, and develop personalized care plans that support quality, compliance, and cost-effective outcomes. Collaborating with an interdisciplinary team, you’ll leverage evidence-based practice, strong documentation skills, and patient education to improve health outcomes.
