Nurse Practitioner ~ Full Time

Join Riverside Home Health & Hospice and make a difference in the lives of patients throughout our community. We are seeking a compassionate and motivated Nurse Practitioner to provide high-quality mobile and community-based healthcare services.
Diagnose and treat acute and chronic conditions
Provide care in homes, Assisted Living Facilities (ALF), and Skilled Nursing Facilities (SNF)
Conduct patient assessments, develop treatment plans, and prescribe medications
Utilize telehealth and in-person visits
Collaborate with patients, families, and healthcare partners
Maintain accurate and timely clinical documentation
Master's Degree from an accredited NP program
Current state licensure and prescribing authority
Valid driver's license and reliable transportation
Strong communication and clinical decision-making skills
Knowledge of HIPAA and healthcare regulations
