Title: Nurse Practitioner (NP) – Remote Care Model | $130K–$170K | Phoenix, AZ

We’re working with a healthcare team focused on managing complex Medicaid populations through a telehealth-first, coordinated care model, designed to improve outcomes while reducing unnecessary hospital utilization.

The role
