Primarily remote / telehealth-based care model

Lead and coordinate care for complex patient populations
Practice with a high level of autonomy, including prescribing
Collaborate with physicians, care managers, and health plans
Support proactive care management and early intervention
Assist patients through transitions of care (hospital to home)
Provide virtual care and guidance to reduce unnecessary ER visits
This is a strong fit for NPs who:
Have 3–5+ years of clinical experience
Are comfortable practicing independently
Have experience with telehealth or remote patient management
Enjoy working with complex populations (Medicaid, elderly, behavioral health)
Thrive in a startup or evolving environment
Want to have a voice in shaping care delivery models
Primarily remote, team-based care model
Mix of W-2 and 1099 options available
$130,000 – $170,000 (depending on experience and leadership level)
Nurse Practitioner license (AZ)
Master’s degree or higher
Strong communication and care coordination skills
