A well-established Community Health Center in San Jose is seeking a dedicated Family Nurse Practitioner (FNP) to join its collaborative care team. Position Highlights

  • Competitive Salary: $158,000 – $185,000 annually

Clinical Support & Resources

  • 1.5 Medical Assistants assigned per provider
  • Dedicated inbox clinicians manage virtual patient communications and message volume
  • Medical Director provides clinical oversight and support
  • Health Center Director manages operational functions
  • Epic EMR platform
  • Team-based approach to patient care
  • Strong collaboration with physicians, behavioral health professionals, nurses, and support staff

Responsibilities

  • Provide comprehensive primary care services, including assessment, diagnosis, treatment, and management of acute and chronic conditions
  • Prescribe medications and develop individualized treatment plans
  • Conduct physical examinations and review patient medical histories
  • Deliver preventive care services and wellness counseling
  • Educate patients on disease prevention, medication management, nutrition, and healthy lifestyle choices
  • Coordinate care with specialists and other healthcare providers
  • Collaborate with interdisciplinary care teams to optimize patient outcomes
  • Maintain accurate and timely clinical documentation in accordance with organizational and regulatory standards
  • Participate in quality improvement initiatives, peer review activities, and team-based care programs
  • Support organizational goals focused on improving access to quality healthcare services

Qualifications

  • Master's Degree or higher in Nursing
  • Current California Nurse Practitioner license in good standing
  • Current Furnishing Number and DEA registration
  • Family Nurse Practitioner (FNP) certification preferred
  • Minimum two years of clinical experience preferred
  • Strong clinical knowledge in family medicine, primary care, internal medicine, and/or pediatrics
  • Knowledge of community-based healthcare delivery and preventive medicine
  • Excellent communication, collaboration, and interpersonal skills
  • Ability to work effectively within a multidisciplinary team environment
  • Commitment to providing compassionate, patient-centered care
