Position Description: We are looking for a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) for our Skilled Nursing Facility ( SNF ) in Springfield, VT for bedside patient care. The provider serves as the primary medical contact for assigned residents, partnering closely with the Medical Director, facility nursing leadership, and interdisciplinary teams. This is a full-time, in-person position with predictable weekday hours and strong work-life balance.

Qualifications:


  • Valid Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Vermont, in good standing

  • Active DEA registration

  • 1+ years of clinical experience; SNF, geriatrics, or primary care preferred

  • Strong clinical documentation skills and familiarity with EMR systems

  • Excellent communication skills with patients, families, and interdisciplinary teams

Responsibilities:


  • Upon admission, examine residents and review their medical records to establish baseline assessments

  • Conduct routine follow-up visits and manage chronic conditions including diabetes, hypertension, CHF, COPD, and dementia

  • Prescribe and adjust medications including controlled substances per state regulations

  • Lead transitional care management and discharge planning with social work, nursing, and rehabilitation teams

  • Engage with residents' families on care plans, goals of care, and advance directives
