Position Description: We are looking for a dedicated Nurse Practitioner in Stow, OH. This is a full-time, in-person position, Monday - Friday 9am - 5pm.
Qualifications of the Nurse Practitioner:
- Active Nurse Practitioner license in the state of Ohio, in good standing.
- Active DEA registration.
- 1+ years of experience as a Nurse Practitioner; SNF, geriatric, or post-acute experience preferred.
- Strong clinical documentation skills and familiarity with EMR systems.
- Excellent communication skills with patients, families, and interdisciplinary teams.
Responsibilities of the Nurse Practitioner:
- Examine residents upon admission and review medical records to establish baseline assessments.
- Conduct routine follow-up visits and manage chronic conditions including diabetes, hypertension, CHF, COPD, and dementia.
- Prescribe and adjust medications including controlled substances per state regulations.
- Lead transitional care management and discharge planning with social work, nursing, and rehabilitation teams.
- Engage with residents' families on care plans, goals of care, and advance directives.
