Nurse Practitioner (Telehealth) - Metabolic Health Program

About the Role:
We are seeking passionate, high-energy Nurse Practitioners (NPs) to join our team and deliver a cutting-edge Metabolic Health Program aimed at reversing mild to moderate metabolic diseases.
This contract-based role is ideal for NPs looking to provide high-impact care to motivated patients who want to improve their health, with the flexibility to set your own schedule and panel size.
You will be an important member of a care team, delivering the PowerME Vitality Program focusing on prevention and early intervention. You will provide low-complexity care focused on the medical management of obesity and metabolic disease using GLP-1 medications (e.g., semaglutide) through telehealth visits (with an option for in-person in some cases).
The program is a behavioral change program, designed to empower patients, offering you the chance to have a meaningful impact in metabolic disease prevention and early intervention.

Key Responsibilities:

• Telehealth Visits: Conduct monthly patient visits (~30 minutes) for medical management, test result reviews, patient education, and motivational support.
• Medical Management: Oversee GLP-1 medications (no other medications required).
• Patient Education & Motivation: Engage patients in understanding their health journey and encourage behavior change towards metabolic health improvement.
• Lab Review & Follow-up: Review lab results and provide guidance and outreach through the patient portal.
• Documentation: Complete simple, template-driven documentation after each visit with consultation letter forwarded to patient's PCP.
• Patient Communication: Respond to minimal patient messaging and medication-related questions.
• Collaboration: Work alongside experienced physicians and other NPs, with support available to you as needed.
• Deliver to Your Own Patients. If you are currently practicing, our program can also be delivered to your own practice.
