Position Summary


The Nurse Practitioner (NP) provides comprehensive, patient-centered primary and urgent care services within a Direct Primary Care (DPC) model. In this role, the NP builds long-term therapeutic relationships with a panel of patients, delivers evidence-based acute and chronic disease management, and supports the DPC mission of accessible, personalized, and cost-effective healthcare. The NP practices with a high degree of clinical autonomy while collaborating with the supervising or collaborating physician as required by state regulations.


Key Responsibilities


Clinical Care & Patient Management


  • Conduct comprehensive health assessments including complete history and physical examinations for patients of all ages (pediatric through geriatric).
  • Diagnose, treat, and manage a broad spectrum of acute illnesses and injuries in the urgent care setting (lacerations, fractures, respiratory infections, UTIs, musculoskeletal injuries, etc.).
  • Provide ongoing chronic disease management for conditions such as hypertension, diabetes, hyperlipidemia, asthma, COPD, and depression.
  • Order and interpret diagnostic studies including laboratory tests, imaging, EKG, and pulmonary function tests.
  • Develop, implement, and adjust individualized patient care plans in partnership with patients and their families.
  • Perform office-based procedures (wound care and suturing, joint aspirations/injections, I&D, cerumen removal, Pap smears, etc.).
  • Provide preventive care services including well-patient exams, immunizations, and health screenings.
  • Prescribe medications and therapies in compliance with state prescriptive authority guidelines and practice protocols.
Direct Primary Care – Membership Model


  • Manage a defined panel of DPC members; ensure timely access through same-day/next-day scheduling and direct communication (phone, text, portal).
  • Facilitate care coordination including specialty referrals, hospital follow-up, and care transitions.
  • Leverage DPC cost-saving tools such as wholesale lab pricing, generic medication dispensing, and negotiated imaging rates to maximize member value.
  • Educate patients on their health conditions, treatment plans, medications, and preventive strategies.
  • Build and maintain trusting, longitudinal relationships that differentiate the DPC model from traditional fee-for-service care.
Documentation & Compliance


  • Maintain accurate, timely, and complete medical records in the electronic health record (EHR) system in compliance with HIPAA and applicable state regulations.
  • Ensure documentation supports medical necessity and meets quality standards.
  • Adhere to OSHA standards, infection control protocols, and clinical safety policies.
  • Participate in quality improvement initiatives, chart audits, and peer review processes.
Collaboration & Team Leadership


  • Work collaboratively with medical assistants, front office staff, and administrative leadership to support smooth clinic operations.
  • Supervise and provide clinical guidance to medical assistants and other support staff as applicable.
  • Participate in staff meetings, case conferences, and continuing education activities.
  • Maintain open communication with the collaborating/supervising physician and escalate complex cases appropriately.
Qualifications


Required


  • Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
  • Current, unrestricted Nurse Practitioner license in the state of practice.
  • National board certification as a Family Nurse Practitioner (FNP-BC or FNP-C) or relevant specialty (Adult-Gerontology, AGNP, PMHNP, etc.).
  • Current DEA registration (or eligibility to obtain).
  • Current BLS/ACLS certification.
  • Compliance with any state-required collaborative or supervisory practice agreements.
  • Work every other weekend
  • Must have a set schedule for continuity in patient care. 
Preferred


  • 1–3+ years of clinical experience in primary care, family medicine, internal medicine, or urgent care.
  • Experience working in a DPC, concierge medicine, or membership-based practice.
  • Proficiency with EHR systems (Elation, Hint Health, Athena, Epic, or similar).
  • Experience with minor surgical procedures and point-of-care testing.
  • Bilingual skills (English/Spanish or other languages) are a plus.

  • Ability to work independently in a fast-paced environment with minimal supervision.
Physical Requirements


This position requires the ability to stand and walk for extended periods, lift up to 30 pounds, and perform fine motor tasks associated with clinical procedures. The NP must be able to work in clinical settings with exposure to infectious diseases, sharps, and medical equipment.
