Community Care Physicians (CCP), the largest independent multispecialty medical group in the Capital Region, is seeking a Nurse Practitioner or Physician Assistant to join Community Care Weight Management and Metabolic Care, a specialized, multidisciplinary program focused on helping patients achieve sustainable weight loss and improved metabolic health.
Position Overview
As an Advanced Practitioner in the Weight Management and Metabolic Care program, you will provide evidence-based care for adult patients with obesity, Type 2 diabetes, and related metabolic conditions. You will work within a highly collaborative, multidisciplinary team-including physicians, dietitians, behavioral health specialists, and clinical pharmacists-to deliver personalized treatment plans that integrate lifestyle modification, medical therapy, and ongoing support.
