Our client, a reputable healthcare provider committed to delivering comprehensive home-based medical services, is hiring a Nurse Practitioner - Home Health. The Nurse Practitioner in this role will be integral to ensuring quality care for homebound patients while coordinating with multidisciplinary teams across Queens and Westchester County. Responsibilities:
- Conduct in-home primary care assessments and treatment for geriatric patients.
- Develop and implement individualized care plans.
- Participate in weekly team meetings to coordinate patient care.
- Maintain detailed documentation of patient interactions and treatment plans.
- Manage and monitor patient panels to ensure continuity and quality of care.
- Utilize telehealth tools as appropriate and coordinate with other healthcare providers.
- Travel within designated regions to facilitate patient visits; transportation stipends or Uber reimbursements available.
Requirements:
- Minimum of 2 years of experience in home care, primary care, or related settings.
- Preferred experience in emergency medicine or ICU if relevant skills are demonstrated.
- Licensed Nurse Practitioner in New York State.
- Strong clinical assessment and decision-making skills.
- Excellent communication and patient engagement abilities.
- Reliable transportation to travel between patient locations; own vehicle preferred, transportation allowance available.
- Ability to work full-time hours, generally 8 AM to 4 PM.
- Comfort with utilizing telehealth technology and electronic health records.
- Bilingual skills are a plus but not mandatory.
