    ## Position Overview Bloom Behavior and Consulting Services is seeking a dedicated, compassionate Nurse to support clients in both our Bloom BCS Clinic and our specialized transition program. The Nurse will:

    • Support student health needs during clinic-based programming.
    • Collaborate with clinical and behavioral staff to ensure safe and consistent care.
    • Support students in our transition program which focuses on vocational development, community engagement, and daily living skills.

    Services occur across school and community settings, helping students build independence and workplace readiness.

    ## Key Responsibilities

    • Provide direct nursing care, first aid, and emergency response to clients/students on site and during community-based activities.
    • Administer medications and maintain accurate medication administration records in accordance with district policy and state regulations.
    • Develop, update, and monitor Individualized Healthcare Plans (IHPs) and collaborate with IEP teams to support student access and safety.
    • Support students with chronic health conditions, mobility needs, seizure disorders, behavioral health needs, and other medically-relevant diagnoses.
    • Train and support staff in safe health procedures, including universal precautions, emergency protocols, and student-specific medical needs.
    • Maintain ongoing communication with families, school staff, and outside providers as appropriate.
    • Participate in crisis intervention and safety-team procedures when needed.
    • Monitor overall health trends within the program and promote wellness practices.
    • May accompany students and staff on community outings and field trips as part of the transition curriculum.

    ## Required Qualifications

    • Current Registered Nurse (RN) license in the State of Connecticut.
    • Current CPR/First Aid certification.
    • Experience working with adolescents and/or young adults preferred.
    • Ability to work collaboratively with multidisciplinary teams (educators, therapists, job coaches, administrators).
    • Strong communication, organization, and clinical decision-making skills.

    ## Preferred Qualifications

    • Experience in special education transition programming or working with individuals with developmental disabilities.
    • Experience supporting students in community-based vocational or life-skills programs.
    • Familiarity with IEP and IHP processes.

    ## Work Schedule

    • Days: Monday through Friday, following the Bloom BCS calendar.
    • Hours: 8:00 AM - 2:45 PM.
