Bloom Behavior and Consulting Services is seeking a dedicated and compassionate School Nurse for The Bloom Transition Program and the Bloom BCS Clinic in Old Lyme. This role provides nursing support during clinic-based programming, collaborates with clinical and behavioral staff to ensure safe and consistent care, and assists with the health needs enrolled in our program.
Key Responsibilities
Provide direct nursing care, first aid, and emergency response to clients and students on site, and during community-based activities as needed.
Administer medications and maintain accurate medication administration records in accordance with district policy and state regulations.
Develop, update, and monitor Individualized Healthcare Plans (IHPs), and collaborate with IEP teams to support student access and safety.
Support students with chronic health conditions, mobility needs, seizure disorders, behavioral health needs, and other medically relevant diagnoses.
Train and support staff in safe health procedures, including universal precautions, emergency protocols, and student-specific medical needs.
Maintain ongoing communication with families, school staff, and outside providers as appropriate.
Participate in crisis intervention and safety-team procedures when needed.
Monitor overall health trends within the program and promote wellness practices.
Accompany students and staff on community outings and field trips as part of the transition curriculum, as needed.
Required Qualifications
Current Registered Nurse (RN) license in the State of Connecticut.
Current CPR/First Aid certification.
Experience working with adolescents and/or young adults preferred.
Ability to work collaboratively with multidisciplinary teams, including educators, therapists, job coaches, and administrators.
Strong communication, organization, and clinical decision-making skills.
Preferred Qualifications
Experience in special education, transition programming, or working with individuals with developmental disabilities.
Experience supporting students in community-based vocational or life-skills programs.
Experience in school setting and/or fast paced enviroment.
Familiarity with IEP and IHP processes.
Work Schedule
Position Type: Full Time
Days: Monday - Friday, following the Bloom Transition Program School Calendar. Opportunity to work summer hours also available.
