We are seeking an experienced and dedicated Nurse Supervisor to lead our Care Management team. Your proficiency in discharge planning and utilization review will be highly valued as you guide our department to achieve exceptional patient care outcomes.

• This candidate will provide Associate Director level supervision to the Care Management Department
• Must have 2 years of experience in discharge planning, utilization review
• Must be CCM & PRI certified
• Bachelor’s degree required but masters preferred

Compensation & Benefits:

• Permanent Salary Range: $115,000 - $140,000 annually

Role Details & Requirements:

• Schedule: 9:00 AM – 5:30 PM (Primarily Monday to Friday; some weekends required)
• Minimum of 2 years in discharge planning and utilization review
• Active CCM & PRI certifications
• Bachelor’s degree required; Master’s degree preferred
