PURPOSE OF THE POSITION:
Provide oversight of nursing services to all individuals in BK, Queens and 1 location in BX.
REPORTS TO: Program Director; Vice President, I/DD Services
SUPERVISES: Nursing Staff
The Nursing Supervisor is responsible for ensuring that all medical treatment of the individuals in the assigned programs receive appropriate and timely medical care in accordance with the regulations of the New York State Office for People with Developmental Disabilities (OPWDD), the Mental Hygiene Law of NYS, and the policies and procedures of SCO Family of Services.
Specific Responsibilities:
· Provides administrative and professional supervision of the nursing staff in each of the assigned residences/sites.
· Develop nursing procedures that reflect current clinical practice, professional standards, and facility philosophy of care
· Ensure provision of nursing and nursing-related services to attain, or maintain, the highest practicable physical, mental, and psycho-social well-being of individuals.
· Communicate and interpret policies and procedures to nursing staff
· Monitor practice for effective implementation
· Establish criteria to assure that services provided meet established standards of quality care; ensure regulatory compliance as set by OPWDD and DOH
· Review the Telehealth services reports as deemed appropriate.
· Compliance with, and communication to, staff of facility policies and procedures for workplace safety, including infection control procedures, application of universal precautions for blood borne pathogens, use of person protective equipment, and handling of hazardous materials.
· Communicate emergency and disaster procedures of facility to nursing staff.
· Conduct, or provide staff to conduct, resident assessments as required; develop plans of care, evaluations; individuals’ response to interventions, and documenting clinical records effectively in compliance with state and federal requirements.
· Establish and implement an infection control program designed to provide a safe, sanitary, and comfortable environment, and to prevent the development and transmission of disease and infection.
· Review infection control surveillance reports to identify trends, and to develop effective actions to control and prevent infections in the facility.
· Communicate directly with individuals, families, medical staff, nursing staff, interdisciplinary treatment team members and department heads to coordinate care and services
· Ensure implementation of plans of care, maintain quality of care and quality of life.
· Review reports daily to ensure timely, effective responses to significant changes in condition, transfers, discharges, use of physical restraints, unexplained injuries, potential abuse or neglect, medication errors; ensure communication to administration of findings and results.
· Participate in the Interdisciplinary Treatment Team to ensure that all team members are aware of any and all medical conditions or concerns.
· Responsible for ensuring communication among and between nursing staff.
· Oversee quality and appropriateness of nursing services, medical doctors, consultants and other professionals.
· Oversee clinical on-site visits
· Ensure physicals, lab work, and doctor’s appointments are arranged as needed.
· Ensure that all medical records, including PONS are kept up to date and meet regulatory standards.
· Ensure Chart Audit Tools are completed by site RN on a quarterly basis to be reviews by Nursing Supervisor.
· Provide direct supervision to all nurses on the roster; ensures that all nursing staff have a valid license as a Registered Nurse in the State of New York.
· Ensure that all nurses on the roster complete and maintain certification in BLS, Individual Rights, Mandated Reporter, Conflict of Interest attestations, and Code of Ethics, and all mandated or assigned LMS training.
· Fill in as an RN as needed; cover for vacant positions to ensure continuity of medical care.
· Review nursing notes and other medical documentation to ensure that it is up-to-date and completed in an accurate and professional manner, minimally monthly; report findings to Nursing Administrator of I/DD or designee.
· Participate in development of policies and procedures relevant to the provision of medical services as needed and requested.
· Ensure pharmacy reviews are conducted; ensure follow-up for all issues identified
· Participate in the Incident Review Committee and Human Rights Committee; other committees and meetings as requested.
· Serve as point person for vendors of medical equipment, medication, etc.
· Provide on-site supervision for nursing staff at all facilities. Completes evaluations in accordance with agency requirements.
· Participate with Human Resources Department to determine progressive disciplinary measures and termination requests as deemed necessary as outlined in SCO's policies and procedures.
· Participate in interviewing, screening, and hiring of nursing staff, ensure initial and on-going training
· Responsible for scheduling and payroll for assigned staff.
· Ensure training and certification for AMAP staff; ensure that all AMAP-certified staff maintain their certification with no lapse.
· Accept special assignments, and ad hoc projects, as requested.
QUALIFICATIONS:
· NYS-licensed Registered Nurse; BSN (MSN preferred)
· Minimum 5 years experience in residential services in OPWDD programs; knowledge of all 624 and 683 regulations
· Minimum 2 years experience in supervisory role
· Must be able to respond to emergencies
· Excellent verbal and writing skills
· Excellent interpersonal skills
· Computer Literate; experience with Electronic Health Record

WORKING CONDITIONS:
· Provide on-site supervision, monitoring, and support to nursing staff and on call for staffing supports 24/7 days a week via agency email/cell phone.
