Job Title: Nurse Supervisor, Health Services (R.N)

Reports To: Director of Nursing

Location: 7 West Burnside Avenue Bronx, New York (Some travel to other Bronx locations will be required)

Hours: 35 hours, Exempt (Onsite role) Hybrid schedules are available based on program needs and at leadership discretions

Salary Range: 90k-100k. Salary commensurate with relevant experience


Job Summary

The Nurse Supervisor is responsible for the daily programmatic oversight of the assigned Care Management unit. This role works closely with Care Managers to ensure the effective provision and coordination of care management services for assigned members.

Major Duties & Responsibilities

Supervision & Leadership

  • Carry an active caseload as required.
  • Provide supervisory support to the assigned Nurse Care Management Team.
  • Conduct individual and group supervision sessions; evaluate supervisees regularly and in a timely manner.
  • Monitor Care Managers’ work with children and families through team meetings and individual check-ins.
  • Demonstrate strong leadership and management skills to support the progressive development of Care Management staff.

Care Management & Program Oversight

  • Provide direct care management services in the absence of Care Management staff.
  • Manage, review, and monitor member records to ensure compliance with agency policies, 29-i regulations, and other oversight requirements.
  • Coordinate audit preparation and execution.
  • Collect, analyze, and present data during supervision to support the Director of Nursing.
  • Oversee the identification and enrollment of eligible members from agency programs and community sources.
  • Assist in developing and strengthening community support networks for families.
  • Work collaboratively with teams across all agency programs.
  • Complete special projects and assignments as requested by the Director of Nursing.
  • Ensure timely completion of all 29-i documentation and regulatory requirements.
  • Perform additional duties as assigned.

Training & Quality Assurance

  • Provide comprehensive training for new staff and ongoing training for existing team members.
  • Ensure quality assurance standards are met internally and externally.

Qualifications

  • Associate Degree in Nursing required; Bachelor’s Degree in Nursing (BSN) strongly preferred.
  • Active and current New York State Registered Nurse License (NYS RN) required.
  • Minimum 2–3 years of direct clinical practice in one or more of the following: Family Health, Pediatrics, or Primary Care. Experience in community-based medical settings is a plus.
  • Minimum 2–3 years of supervisory experience overseeing clinical professionals. Experience in Care Management and Care Coordination highly preferred.
  • Strong knowledge of best practices related to child welfare, developmental disabilities, mental health, and/or primary healthcare systems.
  • Demonstrated administrative skills, including advanced proficiency in Microsoft Excel, Word, Outlook, and experience with Electronic Health Records (EHR) or healthcare data systems.
  • Experience in service coordination, including information sharing, linkage, and referrals to community-based resources.
  • Excellent accountability, written and verbal communication skills, and strong organizational abilities.
  • Compassion and commitment to serving children and families with special needs.
  • Ability to travel to community-based sites as required.
