Key Responsibilities - Monitor and document patient vitals, symptoms, and health status
- Administer and manage medications; pre-fill medication boxes
- Provide care for G-tube feeding, Foley catheters, and suprapubic catheters
- Perform wound care per physician orders
- Manage tracheostomy and ventilator care when applicable
- Provide suctioning and respiratory support as needed
- Maintain accurate and timely charting and documentation in compliance with agency and state standards
- Communicate effectively with patients, families, and interdisciplinary teams
- Maintain a safe and hygienic home environment in line with care protocols
- Provide emotional support to patients and families facing chronic or critical illness
Qualifications - Current LVN or RN license in the State of Texas (required)
- Valid CPR Certification (required)
- At least 1 year of clinical nursing experience (preferred)
- Home healthcare or Private Duty Nursing experience is a plus
- Proficiency in tracheostomy care, ventilator management, G-tube feeding, and catheter care
- Wound care experience (preferred but not required)
- Ability to work independently and manage care plans with confidence
- Excellent communication, assessment, and critical thinking skills
- Willingness to work some evenings, weekends, or holidays as needed
- Reliable transportation and punctuality
