Lead nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience.
Demonstrate proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role model expected behaviors.
Ensure the integration of PHR into clinical activities to enhance Relationship Centered Care.
Validate staff members round hourly on all patients by direct observation and patient feedback.
Assess staff competency in PHR and provide consistent meaningful feedback with more focused feedback as needed.
Oversee that rounding occurs on all patients on shift.
Communicate quantitative and qualitative information regarding the patient experience and staff engagement to the staff and the interdisciplinary health care team that leads to the development and implementation of an action plan.
Collaborate with the physician dyad and escalate concerns as appropriate.
Follow up on all patient concerns in a timely manner and provide service recovery as needed.
Demonstrate caring, respect, compassion, empathy, and active listening through dialogue, body language, and actions.
Requirements:
Bachelor of Science with a major in nursing required; Masters in nursing or health care related field preferred.
Minimum 3 years of clinical nursing experience with some Charge Nurse/supervisory experience preferred.
Licensed as a Registered Nurse with current registration in New York State.
Preferred Skills:
Relevant clinical experience in area of nursing practice assigned.
Ability to maintain and promote interpersonal relationships, communicate appropriately to others, and work effectively and efficiently to solve problems.
Demonstrate the ability to work in a team displaying effective communication and critical thinking.
Ability to read, write, understand, and speak English in a clear and concise manner.
Visual, physical, auditory, and cognitive ability to assess patient status.
Physical and mental health necessary to care for acutely ill patients with a broad variety of diagnoses and behaviors on a daily basis.
Ability to walk, stand, lift, stoop, push, and respond quickly in a variety of situations and for an extended period of time.
Manual dexterity to perform a variety of clinical procedures relevant to practice area (i.e., venipuncture, urinary catheterization, cardiopulmonary resuscitation).
Ability to use data entry, retrieval systems, and the electronic medical record documentation system in a proficient manner.
