We are seeking a compassionate and experienced Bilingual (Spanish) Social Worker to join our team at an 180+ bed skilled nursing facility. The Social Worker will play a key role in supporting residents and their families by addressing psychosocial needs, facilitating care planning, and ensuring a smooth transition throughout the continuum of care. The ideal candidate is patient-focused, organized, and able to work collaboratively in a fast-paced environment.
 

Key Responsibilities:

  • Conduct psychosocial assessments for residents upon admission and throughout their stay
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Provide emotional support, counseling, and crisis intervention to residents and families
  • Assist with discharge planning, including coordination of community resources and services
  • Advocate for residents’ rights and ensure compliance with state and federal regulations
  • Maintain accurate and timely documentation in accordance with facility policies
  • Participate in care plan meetings and family conferences
  • Address concerns related to adjustment, behavioral health, and long-term care placement
  • Support residents and families with Medicaid applications and other financial/social service needs

Qualifications:

  • Bachelor’s or Master’s Degree in Social Work (BSW or MSW required)
  • Bilingual in Spanish and English (required)
  • Prior experience in a nursing home, rehabilitation, or healthcare setting preferred
  • Excellent communication, organizational, and interpersonal skills
  • Ability to work effectively as part of an interdisciplinary team
 

Schedule:

  • Full-time position
  • Participation in a weekend rotation approximately once every 2 months
  • A weekday off will be provided during weeks when a weekend shift is worked
