Help individuals with complex behavioral health and medical needs build stability, strengthen independence, and thrive through compassionate, community-based support.
Schedule: Monday - Friday 8:30am-4:30pm.

The Nursing Home Support Specialist partners with individuals living in Skilled Nursing Facilities (SNFs) and Intermediate Care Facilities (ICFs) who are navigating serious mental illness alongside significant medical needs. Using a rehabilitation and recovery-focused approach, this role provides assertive case management, care coordination, advocacy, and skill-building to help individuals achieve their personal wellness goals, reduce unnecessary hospitalizations, and enhance their quality of life. This position requires regular travel and transportation of clients using a personal vehicle.

Key Responsibilities

  • Build trusting relationships with individuals and provide person-centered, recovery-oriented case management and support.
  •  Complete assessments, develop individualized treatment plans, and monitor progress toward wellness and recovery goals. 
  •  Coordinate behavioral health, psychiatric, medical, and community-based services to ensure continuity of care. 
  •  Advocate for individuals and connect them with housing, social, educational, vocational, and recreational resources. 
  •  Support medication education and adherence while monitoring for concerns or changes in functioning. 
  •  Collaborate with nursing facility staff, healthcare providers, family members, guardians, and community partners to promote positive outcomes. 
  •  Provide supportive counseling, crisis intervention, discharge planning, and ongoing follow-up during transitions of care. 
  •  Coach individuals in daily living, coping, and independent living skills that support long-term success. 
  •  Assist with staff consultation and behavior management strategies within nursing facility settings. 
  •  Maintain timely documentation, required reporting, and compliance with organizational and regulatory standards.

Requirements, Skills, Knowledge and Expertise

  • Bachelor’s degree in counseling, psychology, social work or related mental health field required
  •  A valid, unrestricted driver's license at the time of hire with the ability to obtain and maintain a class E MO driver's license by the end of 60-day introductory period required 
  •  Must obtain and maintain current CPR and First Aid certification within 60 days of hire 
  •  Must obtain and maintain certification in an approved crisis intervention methodology (CPI, MANDT, or CALM) within 60 days of hire 
  •  Must obtain and maintain Medication Level I Aide
