Responsibilities


Woodland Memorial Hospital’s Yolo Adult Day Health Center offers a diverse range of services for individuals and families facing the challenges of serious health problems such as dementia, chronic medical diagnoses, mental illness or brain injury.


The Nursing Supervisor at the Center is responsible for designing, implementing and directing the health maintenance of all program participants.


Essential job responsibilities:


+ Selects, trains, evaluates subordinates and initiates personnel actions in accordance with Human Resources policy and organizational philosophy

+ Supervises the day to day activity of the nursing department, ensuring all departmental and organizational policies and procedures are adhered to and patient concerns are addressed. Responsible for ensuring the department is effective, efficient and operates with a positive sense of teamwork.

+ Establishes and maintains department staffing. Develops and implements staffing levels ensuring appropriate ratios to meet Title 22 regulations.

+ Responsible for various activities on new and active participants including but not limited to: assessment of new participants; completion of six-month reevaluations; completion of quarterly reports and reviewing, care coordination and updating participants medications, diagnosis, and treatments every three months.

+ Prior to enrollment screens all potential participants for meeting medical necessity criteria, ensures completion of all needed medical information including but not limited to current medications, PPD, standing orders and all other physician orders.

+ Ensures medical care is delivered as prescribed by MD orders, recommended by team clinicians and/or established individual plan of care.

+ Confers and collaborates with all members of multidisciplinary team not limited to physicians, rehab therapists, dietitians, social workers, activities, therapy and program aide staff in addition to individuals outside center who are members of participant’s care team.

+ Responsible for ensuring creation of accurate and current schedules for clinical treatment and personal care needs. Oversees dietary orders for accuracy and implementation.

+ Oversees nursing staff daily and quarterly documentation for accuracy and timeliness including completion of all individual plans of care.

+ Ensures nursing supervision for therapy maintenance program in absence of a skilled rehab therapist.

+ Develops annual departmental goals addressing quality assurance, regulatory compliance and patient outcomes.

+ Application of training provided entitled “Person Centered Care in Practice”. Able to articulate and apply model components.

+ Schedules training and presents in-services, as required or needed, to staff, participants, student interns and volunteers.

+ Prepares department for all licensing and certification site visits both scheduled and unscheduled and participates in survey process.

+ Maintains department in compliance with all applicable Dignity Health, local, state, federal and grant requirements including ensuring all clinic certifications are current such as CLIA, disposal of medical waste and quarterly pharmacist review.

Qualifications


Minimum:


+ Associate's Degree in Nursing

+ At least two years RN nursing supervision experience in a healthcare setting

+ Current California RN license

+ BLS certification


We prefer candidates with three years RN nursing supervision experience within a geriatric patient population.
