Position Overview
This position will support patients in improving their health through nutrition education, lifestyle guidance, and care coordination.
The Nutritionist works closely with providers, nursing staff, and care teams to help patients manage chronic conditions such as diabetes, hypertension, and obesity while promoting overall wellness and preventive care.

- Monday 8AM -5 PM
- Tuesday 8AM -5 PM
- Wednesday 8AM -5 PM
- Thursday 8AM -6 PM
- Friday 8AM -3 PM
- No Weekends
Key Responsibilities:
- Provide nutrition education and counseling to patients and families
- Perform basic nutritional assessments and assist with care planning
- Support patients in managing chronic conditions through lifestyle changes
- Collaborate with clinical staff to ensure coordinated, patient-centered care
- Document patient interactions and progress in the electronic medical record (EMR)
- Assist with scheduling, follow-ups, and patient communication
- Participate in team meetings, quality initiatives, and community education efforts
Qualifications:
- Bachelor’s degree in Nutrition, Dietetics, Health Promotion, or related field required
- Healthcare or community-based experience preferred
- Strong communication and organizational skills
- Ability to work both independently and as part of a team
- Experience with EMR systems is a plus
