We are seeking experienced Registered Nurses (RNs) for fully remote Telephonic Clinical Care Manager opportunities supporting Longitudinal and Palliative Care programs.

This is an excellent opportunity for RNs with strong backgrounds in case management, care coordination, palliative care, and telephonic patient support.

KEY RESPONSIBILITIES:

• Provide telephonic clinical care management to assigned members
• Coordinate healthcare services and ongoing plans of care
• Complete comprehensive care management and disease-specific assessments
• Conduct patient outreach based on clinical needs and risk levels
• Provide patient education, health coaching, and treatment-plan support
• Identify and address social determinants of health (SDOH)
• Connect members with appropriate healthcare and community resources
• Coordinate services including therapy, DME, behavioral health, and other clinical resources
• Identify and escalate patient safety concerns
• Participate in interdisciplinary team (IDT) meetings and case discussions
• Maintain accurate and timely clinical documentation

QUALIFICATIONS:

• Active New York State RN license – REQUIRED
• Longitudinal Care, Palliative Care, Hospice, and/or Case Management experience
• Strong care coordination and clinical assessment experience
• Knowledge of Medicare and Medicaid populations
• Telephonic care management experience strongly preferred
• Strong communication, patient advocacy, and problem-solving skills
• Ability to work independently in a fully remote environment

$60/hour
