We are seeking experienced Registered Nurses (RNs) to join our team as Telephonic Clinical Care Managers supporting Longitudinal and Palliative Care programs.

This fully remote opportunity is ideal for an RN with strong case management, care coordination, and telephonic patient support experience who is passionate about helping patients remain safely in their homes and communities.

Key Responsibilities

• Provide telephonic clinical care management to assigned members
• Coordinate healthcare services and ongoing plans of care
• Complete comprehensive care management and disease-specific assessments
• Conduct timely outreach based on patient needs and risk levels
• Provide patient education, health coaching, and support with treatment-plan adherence
• Identify and address social determinants of health (SDOH)
• Connect members with appropriate community and healthcare resources
• Identify safety concerns and coordinate services including therapy, DME, behavioral health, and other clinical resources
• Participate in interdisciplinary team (IDT) meetings and case discussions
• Maintain accurate documentation while ensuring confidentiality and regulatory compliance

Qualifications

• Active New York State RN license – REQUIRED
• Previous Longitudinal Care, Palliative Care, or Case Management experience
• Strong clinical assessment and care coordination skills
• Knowledge of Medicare and Medicaid populations and programs
• Experience providing telephonic patient support strongly preferred
• Excellent communication, problem-solving, and patient advocacy skills
• Ability to work independently and effectively in a fully remote environment

$60/hour
