Description

An OASIS Reviewer ensures the accuracy and compliance of Home Health patient assessments, validates documentation, and supports quality improvement in accordance with CMS guidelines.

Overview
OASIS stands for Outcome and Assessment Information Set, a standardized data set used by the Centers for Medicare & Medicaid Services (CMS) to track patient outcomes, determine reimbursement rates, and monitor quality of care in home health agencies. An OASIS reviewer is typically a registered nurse or other clinical professional responsible for reviewing patient charts and validating that OASIS assessments accurately reflect the patient’s condition

Key Responsibilities
Review and validate OASIS assessments for accuracy and completeness, ensuring compliance with CMS regulations.
Compare clinical documentation (referrals, history and physicals, visit notes, and care plans) against OASIS responses to identify discrepancies or omissions.
Provide feedback to clinicians to improve documentation quality and ensure proper coding.
Collaborate with case managers, clinical staff, and quality assurance teams to monitor performance and support education on OASIS requirements.
Maintain knowledge of CMS guidelines and updates, adapting review processes as regulations change.
Support quality improvement initiatives by identifying trends in documentation errors or compliance issues.

Qualifications
Clinical background: Most positions require a registered nurse (RN) license, though other clinical specialties such as physical therapy may be acceptable.
Experience: Typically, 1–3 years in home health, community health, or acute care settings; experience with OASIS assessments or coding is preferred.
Certifications: Optional but recommended certifications include COS-C (Certificate of OASIS Specialist – Clinical) or COQS, which validate knowledge of OASIS standards 
Skills: Strong attention to detail, analytical skills, ability to interpret clinical documentation, and effective communication for providing feedback to clinicians
