POSITION SUMMARY

The Obstetric Care Nurse Program Coordinator provides comprehensive, patient-centered obstetrics (OB) and maternal care while leading the development, implementation, and ongoing coordination of maternal health programs across the care continuum. The Obstetric Care Nurse Program Coordinator functions as both a clinical navigator and program lead, supporting high-risk and underserved obstetric populations while advancing strategic initiatives that improve access, patient experience, and clinical outcomes. The Obstetric Care Nurse Program Coordinator serves as a liaison across clinical, operational, and community teams and supports organizational priorities including maternal health equity, reduction in Nulliparous, Term, Singleton, Vertex (NTSV) Cesarean (C-section) rates, and enhanced care coordination.

PRIMARY ACCOUNTABILITIES

• Leads the development, implementation, and ongoing coordination of maternal health initiatives, including the Maternity onto Motherhood (MOM) Program, to improve access, patient preparedness, and care continuity across multiple campuses.
• Designs and standardizes prenatal education programming, including in-person, virtual, and group-based formats, to enhance patient experience and support organizational goals such as reduction in NTSV C-section rates.
• Develops and implements workflows for patient referral, engagement, and care coordination, ensuring timely access to services and continuity across outpatient, inpatient, and community settings.
• Collaborates with interdisciplinary teams, including clinical staff, Marketing, Community Benefit, Registration, and Health Information Management (HIM), to support program operations, patient access, and system integration.
• Acts as an Epic Care liaison for maternal health workflows, coordinating with Registration and HIM to create pregnancy episodes for patients receiving care outside of Epic-enabled practices and ensuring accurate input of critical prenatal data, including laboratory results, to support visibility of expected deliveries and continuity of care.
• Aids at system-level in operational processes by improving tracking and visibility of obstetric patients, reducing administrative burden on clinical staff, and enhancing care planning across facilities.
• Monitors and evaluates program performance, including patient engagement, barriers to care, and outcome trends, and contributes to performance improvement and maternal health equity initiatives.
• Identifies and addresses gaps in care delivery, particularly for high-risk and underserved populations, including non-English speaking patients to improve access and culturally competent care.
• Supports community clinics such as Health Department and Brevard Health Alliance (BHA) to coordinate timely and appropriate care delivery for high-risk and low risk maternal levels of care, including referrals, diagnostics, specialty services, and community resources.
• Serves as a central point of contact for patients and community partners, providing navigation, education, and support throughout the perinatal journey; supports performance improvement and maternal health initiatives, including collaboration with Perinatal Safety Nurse initiatives and other organizational priorities.
• Coordinates and provides functional oversight of maternal health education programming and associated educators across facilities, including childbirth, breastfeeding, and newborn care classes, ensuring consistency in curriculum, patient experience, and alignment with organizational standards; supports in the recruitment, onboarding, scheduling coordination, and ongoing of Child Birth Educators.
• Plans, coordinates, and participates in community outreach and engagement activities, including health fairs, baby expos, and community events, to promote maternal health programs, increase patient access, and strengthen community partnerships.

Work Experience

MINIMUM QUALIFICATIONS

• Education: Bachelor's degree in Nursing (BSN), Healthcare Administration, or a relevant field.
• Work Experience: Three (3) years' recent clinical experience in Nursing, or care coordination, specially in obstetrics or maternal health environment.
• Licensure: Registered Nurse (RN) licensure in the State of Florida.
• Certification: American Heart Association Basic Life Support (AHA BLS) Healthcare Provider Completion card prior to start date and maintained.
• Skills/Knowledge/Abilities:
• Ability to coordinate program initiatives and workflows across multiple departments.
• Proficiency in electronic health record systems (e.g., Epic), including pregnancy episode creation and clinical data entry.
• Ability to work autonomously with minimal supervision.
• Strong critical thinking and problem-solving skills.
• Proficiency with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and online teaching platforms (Teams, Zoom).
• Excellent time management and multitasking abilities, with the capability to prioritize effectively.
• Strong written and verbal communication skills, with attention to detail and accuracy.

PREFERRED QUALIFICATIONS

• Education: Master's degree in Nursing (MSN), Healthcare Administration (MHA), or a relevant field.
• Work Experience: Experience with program development, education programming, or community-based health initiatives, collaborating across interdisciplinary teams, including clinical, operational, and community-based partners, supporting or participating in community outreach, health education, or public health initiatives, and providing leadership, coordination, and oversight of programs, initiatives, or interdisciplinary teams.
• Skills/Knowledge/Abilities:
• Ability to proficiently speak both Spanish and English.
• Familiarity with Health Department or correctional care systems and with culturally competent or community-based care models.

PHYSICAL REQUIREMENTS

• Majority of time involves sitting or standing; occasional walking, bending, and stooping.
• Long periods of computer time or at workstation.
• Light work that may include lifting or moving objects up to 20 pounds with or without assistance.
• May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise.
• Communicating with others to exchange information.
• Visual acuity and hand-eye coordination to perform tasks.
• Workspace may vary from open to confined.
• May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
