OBAT Registered Nurse (Full-Time)

About the role
Familias Unidas Outpatient Clinic is a licensed, CARF-accredited behavioral health clinic. 

We provide clinical treatment for mental health, substance use disorder(s) (SUDs), trauma, and other related behavioral health conditions. 

Patients receive: 

• A comprehensive assessment and CasaCare integrated treatment plan. 

• Holistic integrated care from a multi-lingual multidisciplinary team that includes physicians, psychiatrists, clinicians, case managers, and recovery support specialists.

As part of the Casa Esperanza, Inc. team, you will provide services in a fully integrated, holistic, and collaborative manner using a team-treatment approach to ensure the coordination of all client services, both across Casa programs and with external providers.  This integrated model aims to facilitate clear communication amongst all providers and eliminate duplication of efforts or conflicting approaches to care; detail necessarily follow-up and assignments; and ensure wrap-around support services are in place to maximize treatment effectiveness and clinical outcomes.

KNOWLEDGE/SKILLS/ABILITIES:  

• Knowledge of EHR systems  

• Ensure patient confidentiality in accordance with all federal, state, and local laws and regulations  

• Knowledge on best practices associated from the American Society of Addiction Medicine (ASAM) 

• Knowledge of Substance Use Disorder 

• Knowledge of Medication Assisted Treatment for substance use disorders 

Job Duties and Responsibilities: 

Clinical Care Coordination 

• Work as part of an integrated care team model. 

• Monitor patient progress and treatment adherence. 

• Work closely with clinical team during MDT meetings to offer and provide client medical insight as part of case presentations. 

• Participate in multidisciplinary case reviews and treatment planning. 

• Facilitate referrals to higher or lower levels of care as clinically indicated. 

• Coordinate care with primary care providers, behavioral health providers, hospitals, detoxification programs, and specialty providers. 

• Ensure responsibilities regarding Duty to Warn and Mandated Reporting are fulfilled 

• Directly works in collaboration with community providers for medication refills and reconciliation 

Medication-Assisted Treatment (MAT/MOUD)—Nurse Care Manager Model 

• Support prescribing providers in the management of medications for opioid use disorder (MOUD), including: 

• Buprenorphine products 

• Extended-release buprenorphine injections 

• Naltrexone (oral and injectable) 

• Educating clients on their current medical diagnosis, needs, and medical histories; educating clients on medication information and usages 

• Conduct patient initial intake, induction, and stabilization/maintenance OBAT visits. 

• Monitor medication adherence and response to treatment. 

• Educate patients regarding medication use, side effects, storage, and overdose prevention. 

• Monitor for withdrawal symptoms, cravings, and relapse risk. 

Nursing Services 

• Coordinate patient care throughout the OBAT treatment episode. 

• Complete medical intake screenings  

• Conduct nursing assessments and collect medical histories. 

• Complete COWS/CIWA Assessments 

• Obtain vital signs and perform clinical screenings. 

• Conduct medication reconciliation. 

• Administer injectable medications as ordered. 

• Perform venipuncture and specimen collection. 

• Conduct CLIA-waived point of care testing as needed 

• Monitor laboratory results and notify providers of abnormal findings. 

• Work collaboratively with Medical Director/Psych NP for the medication reconciliation process.  

• Keep electronic health records up to date, including progress notes and assessments of service.  

Laboratory and Toxicology Monitoring 

• Coordinate urine drug screening protocols. 

• Collect and process laboratory specimens. 

• Track required laboratory testing, including: 

• CBC 

• CMP 

• Liver Function Tests 

• Hepatitis Screening 

• HIV Screening 

• Pregnancy Testing 

• Other provider-ordered laboratory studies 

• Ensure timely review and follow-up of laboratory results.

While performing the duties of this job, the employee is regularly required to walk, stoop, kneel, crouch or crawl, and talk or hear. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include distance vision, peripheral vision, and depth perception.
