Summary


As an Occupational Therapist at 1st Americare LLC, you will play a vital role in enhancing the lives of our clients through tailored therapeutic interventions. This position is essential in supporting our commitment to delivering compassionate and professional home health care.


Responsibilities


  • Conduct comprehensive assessments of clients' needs and develop individualized treatment plans.
  • Implement therapeutic interventions to improve clients' daily living skills and overall quality of life.
  • Collaborate with a multidisciplinary team to coordinate care and ensure continuity of services.
  • Educate clients and their families about treatment plans and strategies for maintaining independence.
  • Monitor client progress and adjust treatment plans as necessary to achieve optimal outcomes.
  • Document client progress accurately in compliance with regulatory standards.
  • Participate in discharge planning to facilitate smooth transitions for clients leaving skilled facilities or hospitals.
  • Provide training and support to caregivers on therapeutic techniques and client care.
Qualifications


  • Valid Occupational Therapist license in the applicable state(s).
  • Experience in a skilled nursing facility or home health care setting preferred.
  • Strong knowledge of medical terminology and case management practices.
  • Ability to develop effective treatment plans based on client assessments.
  • Excellent communication skills for interacting with clients, families, and healthcare professionals.
  • Understanding of physiology and its application in occupational therapy interventions.
  • Experience with early intervention strategies is a plus.
  • Flexibility to adapt to various client needs and environments.
