Occupational Therapist (OT) – Home Health

Location: Southern Chester County, PA (Team 9)
Facility: Bala Cyndwyd PA
Contract Duration: 13 Weeks
Schedule: Monday–Friday | 8:00 AM – 5:00 PM
Guaranteed Hours: 40 Hours/Week
 

Job Description

Penn Medicine at Home is seeking a skilled and compassionate Occupational Therapist (OT) for a 13-week local contract. The Occupational Therapist will provide in-home therapy services to patients, helping them regain independence and improve their ability to perform daily living activities.

Responsibilities

  • Evaluate patients in their homes and develop individualized treatment plans.
  • Provide occupational therapy services to improve functional independence and quality of life.
  • Train patients in activities of daily living (ADLs), adaptive equipment, and safety techniques.
  • Assess home environments and recommend modifications to improve patient safety.
  • Educate patients and caregivers on treatment plans and home exercise programs.
  • Maintain accurate and timely clinical documentation in the EMR.
  • Collaborate with physicians, nurses, physical therapists, speech therapists, and other healthcare professionals.
  • Ensure compliance with all Penn Medicine, state, and federal regulations.

Requirements

  • Active Occupational Therapist (OT) license in Pennsylvania.
  • Graduate of an accredited Occupational Therapy program.
  • Minimum 1–2 years of recent Occupational Therapy experience (Home Health experience preferred).
  • Current AHA BLS Certification.
  • Valid driver's license, reliable transportation, and auto insurance.
  • Strong communication, documentation, and patient care skills.
