Cheraw Post Acute is seeking a dedicated and compassionate Full-Time Occupational Therapist (OT) to join our exceptional in-house therapy team. PRN opportunities are also available.

Facility: 132-Bed Skilled Nursing Facility (SNF)

Responsibilities

  • Assess patients' physical abilities and limitations through comprehensive evaluations.
  • Develop individualized treatment plans based on patient needs and goals using evidence-based practices.
  • Implement therapeutic exercises and interventions to improve mobility, strength, and overall function.
  • Educate patients and families regarding treatment plans, home exercise programs, and self-management strategies.
  • Collaborate with interdisciplinary team members to ensure comprehensive patient care and effective discharge planning.
  • Maintain accurate and timely documentation of patient progress, treatment interventions, and outcomes in compliance with regulatory standards.
  • Stay current with advancements and best practices in occupational therapy.

Qualifications

  • Master of Occupational Therapy (MOT) or Doctor of Occupational Therapy (DOT) degree from an accredited program.
  • Current state licensure or eligibility for licensure as an Occupational Therapist.
  • Strong knowledge of medical terminology and occupational therapy practices.
  • Experience in a skilled nursing facility (SNF) setting preferred but not required.
  • Familiarity with discharge planning and transitional care processes.
  • Experience treating geriatric populations is a plus.
  • Excellent communication and interpersonal skills.
