Job Summary
The Occupational Therapist provides evaluation and treatment services to residents in a skilled nursing facility setting, focusing on improving independence in daily living activities and overall functional abilities. This role supports residents recovering from illness, injury, or chronic conditions.
Key Responsibilities
- Evaluate residents’ physical, cognitive, and functional abilities
- Develop individualized treatment plans to improve activities of daily living (ADLs)
- Provide therapeutic interventions to improve mobility, strength, coordination, and cognition
- Train residents in adaptive techniques and use of assistive devices
- Collaborate with nursing staff, physicians, PT/SLP, and interdisciplinary care teams
- Document evaluations, progress notes, and discharge summaries in compliance with facility standards
- Educate residents and families on home exercise programs and adaptive strategies
- Participate in care planning meetings and rehabilitation team discussions

- Fast-paced, interdisciplinary rehab setting
- Focus on geriatric and medically complex residents
Schedule
- Part-time
- Per diem (as needed / flexible shifts)
Typical Requirements (based on similar Nexus OT postings & IL standards)
- Bachelor’s or Master’s degree in Occupational Therapy
- Active Illinois Occupational Therapist license
- NBCOT certification (commonly required in Illinois rehab roles)
- CPR certification (often required in skilled nursing settings)
- Prior experience in rehab, SNF, or acute care preferred
