Falls Village Rehabilitation & Wellness is seeking a compassionate and dedicated PRN Occupational Therapist (OT) to join our in-house rehabilitation department. This is an excellent opportunity for an OT looking for flexible weekday hours while making a meaningful impact in the lives of residents in a skilled nursing and long-term care setting.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans
• Provide evidence-based therapy services to maximize functional independence
• Accurately document evaluations, treatments, and progress using Net Health and PCC
• Collaborate with physicians, nursing staff, therapists, and interdisciplinary team members
• Educate residents, families, and caregivers regarding treatment plans and discharge recommendations
• Maintain compliance with all state, federal, and facility regulations

Qualifications

• Current Ohio Occupational Therapist (OT) license in good standing
• Skilled nursing or long-term care experience preferred, but motivated new graduates are encouraged to apply
• Strong communication, clinical, and organizational skills
• Experience with Net Health and PCC is a plus
