Shinnery Oak Community is seeking a skilled and compassionate Occupational Therapist (OT) for a per diem (PRN) opportunity within our in-house subacute rehabilitation program. This role offers flexibility while allowing you to make a meaningful impact on the lives of our residents.


Job Summary

The Occupational Therapist (OT) is responsible for evaluating, planning, and implementing individualized treatment programs for residents in a subacute and long-term care setting. The OT works collaboratively with an interdisciplinary team to help patients achieve their highest level of independence and function.


Responsibilities

  • Evaluate residents and develop individualized treatment plans based on physician orders
  • Provide skilled occupational therapy services in accordance with established plans of care
  • Assess patient progress and adjust treatment plans as needed
  • Document evaluations, treatments, and patient progress in compliance with company policies and regulatory standards
  • Collaborate with physicians, nursing staff, and rehabilitation team members
  • Educate patients, families, and caregivers on therapeutic techniques and care plans
  • Ensure all services are delivered in accordance with state and federal regulations
  • Participate in care plan meetings and interdisciplinary team discussions as needed

Qualifications and Skills

  • Current Occupational Therapist (OT) license in the state of Texas required
  • SNF or subacute/long-term care experience preferred
  • Strong clinical assessment and treatment planning skills
  • Knowledge of current therapy practices and regulatory guidelines
  • Experience with electronic documentation systems (Net Health preferred)
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
