Job Overview
We are seeking a compassionate and motivated Occupational Therapist to join an interdisciplinary care team supporting frail and elderly participants within a PACE (Program of All-Inclusive Care for the Elderly) model. The OT will focus on restoring, maintaining, and improving participants' ability to perform daily activities and maximize independence.
This role emphasizes long-term patient relationships, collaborative care planning, and a balanced workload with manageable patient volumes.
Key Responsibilities
- Conduct occupational therapy assessments in facility and home settings
- Develop individualized treatment plans in collaboration with the interdisciplinary team (IDT)
- Provide interventions focused on ADLs, functional mobility, and independence
- Deliver and document therapy interventions according to care plans
- Train and guide therapy assistants in implementing Functional Maintenance Programs
- Participate in interdisciplinary team meetings and care planning
- Support participants in improving strength, coordination, and daily function
- Ensure compliance with clinical standards and regulatory guidelines
Requirements
- Active California Occupational Therapist License (required)
- Bachelor's degree in Occupational Therapy (Master's preferred)
- Minimum 1 2 years clinical experience (geriatrics preferred)
- Experience in outpatient, acute, inpatient, or skilled nursing settings preferred
- CPR and First Aid certification (required within 180 days of hire)
- Ability to work in clinic and community/home-based environments
- Strong communication and patient-centered care skills
