Occupational Therapist

Role Summary

Are you a passionate therapy professional looking to escape high-volume burnout and make a meaningful, lasting difference? We are seeking a dedicated Occupational Therapist to join our mission-driven, value-based healthcare team. In this deeply rewarding role, you will leave behind the frantic pace of seeing three to four patients an hour. Instead, you will have the time and autonomy to build long-term relationships with a manageable panel of vulnerable seniors, focusing on comprehensive, restorative care. Working side-by-side with our staff physicians and a collaborative team of clinicians, you will design and deliver personalized rehabilitative plans that enhance daily living performance, safety, and independence. If you want a role that balances clinical impact with an unrivaled work-life balance and zero after-hours documentation, we invite you to lead our therapy department!

Duties & Responsibilities

Holistic Assessment & Treatment

  • Comprehensive Evaluations: Conduct thorough occupational therapy assessments within our state-of-the-art facility and occasionally within patients' homes (typically 1–2 days per week) to establish clear care goals, preferences, and benchmarks.

  • Therapeutic Interventions: Deliver expert treatments including active and passive exercise, muscle re-education, gait training, and functional training for activities of daily living (ADLs).

  • Equipment & Environmental Safety: Evaluate patient environments and recommend specialized adaptive devices or techniques to maximize safe, everyday independence.

  • Collaborative Care Planning: Actively participate in clinical team meetings to share diagnostic insights, establish unified benchmarks, and develop deeply personalized care blueprints.

  • Staff Mentorship: Work closely with Occupational Therapy Assistants (COTAs), providing the clear training, instruction, and guidance necessary to successfully implement tailored Functional Maintenance Programs (FMPs).

  • Compliant Documentation: Deliver and document all clinical interventions in real time following established department guidelines, ensuring your administrative work is fully completed within standard shift hours.

  • Other duties as assigned.

Required Qualifications

  • Licensure: Valid, active California Occupational Therapist License.

  • Education: Bachelor’s degree in Occupational Therapy from an accredited program.

  • Life Support: Ability to obtain CPR and First Aid certifications within 180 days of hire (certifications in hand upon hire preferred).

  • Mobility Alignment: Flexibility and willingness to travel into patient homes 1 to 2 days per week for home-based safety assessments and care delivery.

Desired Qualifications

  • Advanced Degrees: Master’s degree or higher in Occupational Therapy is preferred.

  • Geriatric Background: Two (2) or more years of experience in a clinical setting working directly with a frail, elderly, or specialized senior population.

  • Clinical Versatility: Previous hands-on experience across outpatient, acute care, inpatient rehabilitation, or skilled nursing environments.

Location and Work Type

  • Work Type: Full-Time, Permanent Position.

  • Work Setting: A refreshing blend of on-site clinical facility care and community-based home visits 1 to 2 days per week (dedicated office space is available on-location).

  • Schedule: Consistent, stable daytime schedule providing an exceptional alternative to traditional, high-burnout acute rehab structures.

  • Location: Fresno, California

Compensation, Benefits & Perks

  • True Work/Life Balance: Enjoy 17 days of Personal Time Off (PTO), 12 paid holidays observed annually, and 6 designated sick days.

  • Comprehensive Healthcare: Immediate access to premium medical, dental, and vision insurance coverage options.

  • Financial Security: 401(k) retirement savings program equipped with a competitive company match.

  • Total Rewards: A robust total compensation package including stable base pay, performance bonus opportunities, and continuous professional development paths.
