Performs Occupational Therapy evaluation, reassessment and consultation; provides plan of care, discharge planning, patient, and significant other education; renders occupational therapy services within the practice guidelines of the American Association of Occupational Therapy (AOTA) and other regulatory agencies.
Essential Duties and Responsibilities:
Performs Occupational Therapy evaluation, reassessment and consultation upon referral of a physician, dentist or podiatrist.
Provides plan of care and discharge planning. Secures a physician certificate for the plan of care.
Provides Occupational Therapy services within the practice guidelines of the AOTA, the Illinois Occupational Therapy Practice Act and other regulatory agencies.
Participates in discharge planning, when requested.
Communicates with patient’s physician, nurse and case manager when necessary regarding the patient’s evaluation report, progress and discharge plan.
Ensures patient safety, patient rights and HIPAA compliance.
Required Qualifications
Bachelor of Science or Master of Science in Occupational Therapy.
