Occupational Therapist - Home Health

Help patients regain independence and improve their quality of life from the comfort of their homes. We’re seeking a compassionate and dedicated Home Health Occupational Therapist to provide personalized therapy services across our home health and hospice communities.

What You’ll Do:

• Evaluate patients and develop individualized occupational therapy treatment plans

• Provide therapeutic interventions to improve physical, cognitive, and daily living skills

• Collaborate with interdisciplinary healthcare teams to support patient recovery goals

• Establish and coordinate care and discharge plans for continuity of treatment

• Complete accurate and timely clinical documentation

• Supervise therapy assistants and support staff as assigned

What You’ll Bring:

• Current Occupational Therapy license in applicable state(s) of practice

• Master’s Degree in Occupational Therapy or Doctor of Occupational Therapy (OTD) required

• BLS/CPR certification required

• Valid driver’s license and reliable transportation required

• Strong communication, organization, and patient care skills

• Passion for helping patients achieve independence in their home environment
