    At the direction of the Clinical Manager, the Occupational Therapist screens, evaluates, re-evaluates and treats patients referred by a licensed provider. Coordinates and communicates treatment activities with health care team members and the patient. Works closely with the interdisciplinary team to ensure high-quality patient care.

    Responsibilities

    The Occupational Therapist:

    Evaluates the patient''s need for occupational therapy and develops/carries out appropriate treatment plan.Instructs family members, COTA''s, Home Health Aides, and other caregivers in appropriate physical therapy techniques.Communicates with the physician and other health care providers on a regular basis regarding the patient''s progress.Provides clinical oversight of COTAs in the home.

    Qualifications

    REQUIRED QUALIFICATIONS

    • Must be a licensed Occupational Therapist in Vermont and New Hampshire or hold a compact license.
    • Graduate of an accredited school in Occupational Therapy

    PREFERRED QUALIFICATIONS

    • One year of experience as an OT in a home health care setting is preferred

    Required Licensure/Certifications

    • Unencumbered OT license in VT and NH or compact.
    • Driver's license, minimum requirements on car insurance, and a clean driving record
