Occupational Therapist - Indianapolis, IN 

Pride Health is hiring Occupational Therapists (OT) for a Home Health contract opportunity in the Indianapolis, IN area.

Job Details

• Location: Indianapolis & surrounding areas
• Schedule: Monday–Friday | 8:00 AM – 4:30 PM
• Weekly Hours: 36 hours guaranteed
• Mileage Reimbursement: Yes (between patient visits)
• On-Call: None
• Contract: Travel/Contract
  
  Pay -$51 to $54/ hr on W2 

Responsibilities

• Complete Start of Care (SOC) visits
• Perform Recertifications and Discharge visits
• Assess patients and develop individualized care plans
• Provide skilled care and patient/caregiver education
• Maintain accurate clinical documentation
• Collaborate with the interdisciplinary care team

Requirements

• Active OT license
• Home Health experience required
• Strong communication and patient care skills
• Valid driver's license and reliable transportation
