Worksite Type: Fully On‑Site 

Location: Cordova, TN 

Employment Type: Contract 

Schedule: Monday–Friday, 8:00 AM–4:30 PM 

Weekly Hours Guaranteed: 36 

Relocation: Not provided 

Overview

We are seeking a dedicated Occupational Therapist to provide high‑quality home health services across multiple counties. This role focuses on delivering skilled assessments, treatments, and patient support in a home‑based setting.

Key Responsibilities

• Perform skilled assessments, reassessments, treatments, and discharge planning for home health patients

• Complete OASIS documentation

• Deliver patient‑centered care across assigned coverage areas

• Collaborate with interdisciplinary care teams to support patient progress and safety

Coverage Areas

• Shelby County

• Fayette County

• Tipton County

Required Qualifications

• Bachelor’s degree

• Previous home health experience

• Strong clinical assessment and documentation skills

Preferred Skills

• Experience with HCHB (Homecare Homebase)

Additional Details

• On‑call/weekend requirement: None

• Mileage reimbursed between visits (from first patient to last)
