We are seeking a dedicated Occupational Therapist to provide high‑quality home health services across multiple counties. This role focuses on delivering skilled assessments, treatments, and patient support in a home‑based setting.
Perform skilled assessments, reassessments, treatments, and discharge planning for home health patients
Complete OASIS documentation
Deliver patient‑centered care across assigned coverage areas
Collaborate with interdisciplinary care teams to support patient progress and safety

Previous home health experience
Strong clinical assessment and documentation skills
Experience with HCHB (Homecare Homebase)
On‑call/weekend requirement: None
Mileage reimbursed between visits (from first patient to last)
