Bargaining Unit: Non Represented

Rate of Pay: $62.77/hour + DOE ($55.80 Base Rate includes 12.5% per diem premium)

Summary
Under the direct supervision of the director, the Home Health Occupational Therapist (OT) is responsible for providing patient care services consisting of evaluation and implementation of modalities and treatments in accordance with established departmental policies and procedures and as indicated by licensing agencies in the State of practice (CA/NV) in the home setting. Therapy Services provides clinical supervision and collaborates with Home Health in evaluating clinical competencies and Home Health maintains responsibility for operational oversight and management.

Essential Duties and Responsibilities
  • Organizes and conducts medically prescribed Occupational Therapy services including evaluation and treatment in the home setting.
  • Effectively utilizes appropriate resources to ensure sensitive, personal care to individuals who require occupational therapy services in the home setting.
  • Plans individualized therapy programs for each patient in the home setting..
  • Collaborates with other team members and services in the Home Health team to assure that services and activities can be properly maintained to meet the needs of the patients.
  • Evaluates, records, and reports patient progress utilizing EMR OASIS documentation in the home setting.
  • Assures that patients, care takers, and families follow established policies and procedures at all times.
  • Interprets occupational therapy policies and procedures to patients, care takers, and family members, as necessary.
  • Remains current in Occupational Therapy practices.
  • Facilitates ordering and training to durable medical equipment (DME) as needed in the home setting.
  • Performs and documents home safety assessments.
  • Participates in peer feedback to improve performance of line of service.
  • Clinician will practice fiscal responsibility and strive to maintain minimum targeted key performance indicators as set by Director and Clinical Manager of department.
  • Extends courtesy and consideration to others in interpersonal relationships.
  • Demonstrates a professional manner in the workplace at all times.
  • Seeks clarification of duties when needed.
  • Channels suggestions, feedback, and questions to appropriate personnel.
  • Participates in the quality improvement process.
  • Establishes good rapport with all team members, patients and other health care providers.
  • OT will participate in activities to meet the health needs of people locally, nationally, or globally.
  • Performs Hospice evaluation/treatment as ordered.
  • Assists with patient care in other clinical settings as assigned.
  • Demonstrates System Values in performance and behavior.
  • Complies with System policies and procedures.
  • Other duties as may be assigned.

Minimum Education/Experience
Bachelor's Degree from an accredited  occupational therapy program, be licensed to practice and pass the National Board of Certification in Occupational Therapy (NBCOT) exam

Required Licenses/Certifications
Basic Life Support (BLS) for Healthcare Providers
  • Upon hire
BLS: Employee will be enrolled in the Resuscitation Quality Improvement (RQI) Basic Life Support (BLS) Entry or Prep Curriculum (depending on their previous BLS certification).
  • Within 3 months of hire into job
Other Experience/Qualifications
Required:
  • California Occupational Therapy license
  • Nevada Occupational Therapy license
Preferred:
  • Masters or Doctorate degree in related field
