Occupational Therapist | Home Health & Hospice
Help patients regain independence and improve quality of life—right where they feel most comfortable. We’re seeking a compassionate and skilled Occupational Therapist to join a dedicated Home Health & Hospice team serving communities across the West Coast.
What You’ll Do:
Evaluate patients and develop individualized treatment plans
Provide occupational therapy interventions to improve daily living skills and mobility
Collaborate with interdisciplinary care teams to support patient goals and recovery
Complete accurate clinical documentation and discharge planning
Supervise therapy assistants and support quality patient care
What You’ll Bring:
Master’s or Doctorate degree in Occupational Therapy required
Active Occupational Therapy license in applicable state(s)
BLS/CPR certification required
Valid driver’s license and reliable transportation required
Strong communication, documentation, and patient care skills
