Occupational Therapist | Home Health & Hospice

Help patients regain independence and improve quality of life—right where they feel most comfortable. We’re seeking a compassionate and skilled Occupational Therapist to join a dedicated Home Health & Hospice team serving communities across the West Coast.

What You’ll Do:

  • Evaluate patients and develop individualized treatment plans

  • Provide occupational therapy interventions to improve daily living skills and mobility

  • Collaborate with interdisciplinary care teams to support patient goals and recovery

  • Complete accurate clinical documentation and discharge planning

  • Supervise therapy assistants and support quality patient care

What You’ll Bring:

  • Master’s or Doctorate degree in Occupational Therapy required

  • Active Occupational Therapy license in applicable state(s)

  • BLS/CPR certification required

  • Valid driver’s license and reliable transportation required

  • Strong communication, documentation, and patient care skills
