Position Summary• Responsible for evaluating, establishing and implementing the plan of care to restore function and prevent disability following disease, injury or loss of a body part.

Education• Bachelor’s degree in Occupational Therapy

Licenses and Certification• Illinois license in Occupational Therapy or eligible• BLS

Experience and Skills• Technical Experience: N/A

Role Specific Responsibilities• Performs thorough evaluation• Develops appropriate Plan of Care• Implements Plan of Care• Documentation is accurate, complete and timely• Provides supervision to assistants, rehab techs, and students
