Job Summary and Qualifications The Occupational Therapist provides high-quality patient-centered care by managing the complete process of occupational therapeutic care of an assigned group of patients in coordination with the physician and the patient care team.
The Occupational Therapist completes patient assessments and participates in developing and implementing individualized care plans focusing on goal achievement, patient engagement, and positive patient experience.
The Occupational Therapist provides or directs a majority of the patients physical therapy to support patients throughout home health and the continuum of care.
What you will do in this role:
• Demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patients requirements relative to his/her age-specific needs. • Demonstrate knowledge to assess and adjust care in coordination with the healthcare team across the continuum. • Assess patients and families ability to learn and incorporate teaching throughout hospitalization and in preparation for discharge. • Deliver compassionate, holistic care while supporting the rights and dignity of all patients.
Qualifications State of Tennessee Occupational Therapist license required. Must be a graduate of an accredited OT program. Basic Life Support (BLS) certification current.
