AllStar Therapies is seeking a compassionate and dedicated Home Health Occupational Therapist (OT) to provide exceptional in-home therapy services. This rewarding position offers the opportunity to help patients regain independence and safely remain in their homes while delivering one-on-one, patient-centered care.

Responsibilities

• Evaluate patients in their homes and develop individualized treatment plans

• Provide skilled occupational therapy services in accordance with physician orders and the established plan of care

• Assess and improve patients' ability to perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs)

• Recommend adaptive equipment, home modifications, and safety strategies

• Educate patients, families, and caregivers on treatment plans and home exercise programs

• Complete timely and accurate documentation in accordance with agency, state, federal, and Medicare guidelines

• Communicate effectively with physicians, nurses, case managers, and the interdisciplinary care team

• Participate in care conferences and discharge planning as needed

Qualifications

• Current Occupational Therapist (OT) license in the state of practice

• Graduate of an accredited Occupational Therapy program

• Home health experience preferred, but not required

• Valid driver's license, reliable transportation, and current auto insurance

• Ability to work independently while managing a flexible daily schedule

• Excellent communication, documentation, and organizational skills

• CPR certification preferred
