Position Overview

A Home Health Occupational Therapist is needed to provide high‑quality, patient‑centered therapy services in a travel/field‑based role. This position supports patients in their homes, delivering evaluations, treatments, and documentation within an EMR system. The therapist will manage a regional caseload and ensure timely, accurate clinical care.

Key Responsibilities

• Perform occupational therapy evaluations, Start of Care visits, recertifications, and treatment sessions.

• Deliver therapeutic interventions to improve patient function, safety, and independence.

• Document all care accurately and efficiently within the EMR (HCHB).

• Collaborate with interdisciplinary care teams to support patient goals and care plans.

• Travel between patient homes within assigned counties; mileage reimbursed between first and last patient.

• Maintain productivity expectations of approximately 30 points per week.

• Follow all regulatory, clinical, and organizational standards.

Required Qualifications

• Active Occupational Therapist (OT) license in the state of Ohio.

• Minimum 1 year of home health experience.

• Proficiency with HomeCare HomeBase (HCHB) documentation.

• Ability to work independently in a field‑based environment.

• Completion of required occupational health screenings.

Preferred Skills

• Experience or certification in Lymphedema Therapy is a strong plus.

• Strong communication, time‑management, and clinical assessment skills.

Schedule

• Shift: Monday–Friday

• Hours: 8:00 AM – 4:30 PM

• On‑Call/Weekend: Not required
