Job Title: Occupational Therapist

Job Location: Bourne, MA
Job Duration: Approx. 3+ months, with possibility of extension
Weekly Hours: 32 hours guaranteed
Schedule: 5 days/week | 8:00 AM – 4:30 PM

Job Description:

We are seeking a Occupational Therapist to provide home health services in the community and assisted living settings. The Occupational Therapist will be responsible for delivering high‑quality patient care through evaluations, treatments, and care planning to support patient independence and quality of life.

Key Responsibilities

• Perform occupational therapy start of care visits, evaluations, recertifications, discharges, and treatments
• Provide therapy services in community and assisted living environments
• Develop and implement individualized plans of care
• Monitor patient progress and update documentation accordingly
• Complete accurate and timely clinical documentation per guidelines
• Collaborate with interdisciplinary care teams to support patient outcomes

Required Qualifications

• Active Occupational Therapy license in Massachusetts
• Home Health experience
• Experience with Homecare Homebase (HCHB)
• Bachelor’s degree in occupational therapy
• Ability to meet occupational health screening requirements

Preferred Qualifications

• Experience with cognitive therapy
• Strong documentation and time‑management skills

Additional Details

• Coverage includes Bourne territories, South Shore, South Coast, and Cape Cod regions
• Mileage reimbursed between patient visits (per policy)
