NOW HIRING an Occupational Therapist for per diem opportunities in a home care setting. You will be paid for each completed patient visit (with documentation).
WORK INDEPENDENTLY * 1-to-1 IN-HOME PATIENT CARE * NO OASIS! (We have RNs for that)

RESPONSIBILITIES:
- Perform home visits for patients in a home care setting in Windham and/or Tolland Counties (New London county also possible)
- Regularly re-evaluates the therapy needs of our clients; initiates, develops, implements, and makes necessary revisions to our patient's plan of care
- Observes, records, and coordinates care with multi-disciplines regarding the patient's treatment and any change in condition
- Teaches, supervises, and counsels the patient and family regarding home therapy procedures and other care needs as appropriate to the patient’s condition
QUALIFICATIONS:
- Current license as an Occupational Therapist in Connecticut
- Valid driver’s license
- Reliable transportation and willing to travel within a designated service area
- Strong communication and interpersonal skills
- Home health experience including Medicare helpful but not required
