Location: Fair Oaks Health Care Center– Crystal Lake, IL (on-site position) 
Status:  Part-Time

Job Summary: 
The Occupational Therapist in our rehab/long-term care setting is responsible for evaluating residents, developing and implementing individualized plans of care, and providing ongoing treatment to support functional independence and quality of life. This role includes completing timely and accurate documentation, reassessing progress, and modifying treatment plans as needed to meet regulatory requirements. The Occupational Therapist collaborates closely with the COTA and interdisciplinary team to coordinate care and discharge planning, delegates and supervises therapy services in accordance with state practice standards, ensures safe and effective treatment through appropriate use of precautions and modalities, and maintains compliance with state, federal, and billing regulations. Participation in required training and continuing education is expected.

Compensation:     $45 to $52 per hour (depending on experience)

Key Responsibilities: 
  • Performs initial patient examination, evaluation, interpretation of referrals, and creates the initial plan of care for each patient, revises as appropriate. 
  • Creates and maintains a patient record for each patient treatment. 
  • Provides on-site assessment and re-evaluation of each patient's treatment according to the State and Federal required interval, whichever is more stringent and adjusts the treatment plan as appropriate.
  • Coordinates discharge plan and decision with the COTA. 
  • Demonstrates knowledge and appropriate application of precautions and contraindications to occupational therapy treatment procedures and modalities.. 
  • Completes documentation according to facility policies including timeliness and medical necessity support. 
  • Delegates appropriate portions of the treatment plan and program to the COTA consistent with the COTA's education, training, and experience.
  • Provides supervision of COTA as required by State Practice Act.
  • Adheres to state, federal and third-party billing regulations.
  • Maintains confidentiality, promotes and supports compliance by acting consistently through Facility Code of Conduct and leading by example.
  • Participates in in-service training and completes all mandatory training within designated time frame.

Qualifications: 
Required: 
  • Graduate of an accredited educational program in occupational therapy.
  • Licensed in the State of Illinois
  • Must not be excluded or sanctioned from state or federal program
  • Thorough knowledge of the principles, methods, materials and equipment used in the practice of their discipline.
  • Ability to use analytical skills to develop outcome solutions.
  • Prior EMR/computer experience
  • Ability to meet physical and job-related requirements 
  • Must be English literate and possess excellent written and verbal communication skills 
Preferred: 
  • Previous hospital or long-term care or skilled nursing experience
