Falls Village Rehabilitation & Wellness is looking for a Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team in Cuyahoga Falls, Ohio.

What You’ll Do

As a Full-Time Occupational Therapist, you will:

• Evaluate residents and develop individualized plans of care
• Provide skilled occupational therapy services in a SNF/LTC setting
• Help residents improve independence with activities of daily living, mobility, safety, and functional skills
• Collaborate with PT, SLP, nursing, and other members of the interdisciplinary team
• Educate residents, families, and caregivers
• Complete accurate and timely clinical documentation
• Participate in care planning and help residents work toward meaningful functional goals

What We’re Looking For

• Graduate of an accredited Occupational Therapy program
• Current or eligible Ohio OT license
• Strong communication and teamwork skills
• A positive, patient-centered approach to care
• SNF/LTC experience is a plus, but new graduates are absolutely welcome to apply!
• Experience with Net Health and PointClickCare (PCC) is a plus
